All patients must complete a Health History and Patient Information form prior to their appointment. Digital intake helps your provider to understand your health history, and current symptoms ahead of time. Please make sure you complete all sections of the digital intake to ensure we can check your insurance benefits prior to your visit.

Proactive Rehabilitation & Fitness provides “Good Faith Estimates” (GFE) of our costs to anyone who does not have health insurance or chooses to pay cash rate. Please request the information or schedule services at least 3 business days in advance.
